An excellent opportunity has arisen to come and be part of our dynamic Occupational Therapy service. As a service we have developed innovative ways of delivering evidenced based and outcome measured occupational therapy in the Older People Service to support people in their individual recovery journey, including a variety of occupationally focused group programmes.
The post will include delivering both individual and group occupational therapy interventions within a clear care pathway, these interventions include:
- Lifestyle Matters Programme – a 20 session group which utilises Lifestyle Redesign© principles.
- Memory Matters Programme – a 6 session group for people with early stages of dementia and their carers.
- Comprehensive occupational therapy assessments & Individual occupational therapy interventions.
We use Model of Human Occupation to guide all our occupational therapy assessments and interventions. We are particularly focusing on embedding occupational science and neuroscience into our practice. The Occupational Therapy Leadership team is committed to an extensive, coordinated CPD programme including Trust wide training, core competence training, annual conferences, robust supervision structure, specialist interest groups and Older People’s care groups. Staff are valued for their expertise in occupationally focused interventions and specialist group therapy.
Main duties of the job
The North West Sector comprise of four Community Mental Health Teams who are based at Unither House in Chertsey. The Community Mental Health Teams for Older People support people who are over 65 with functional mental health conditions or dementia. Your role within the NW sector would be to provide high quality occupational therapy assessments, individual and group interventions across the four teams and support the wider health and social care system in understanding the occupational needs of service users and their families/carers. You will be working with another Occupational Therapist in delivering the Occupational Therapy service.
