An exciting opportunity has arisen for 2 x Band 6 Nurses fixed term for 12 months to work as a Link Worker with our Psychiatric Liaison Team.
The successful candidates will be the responsible Link Workers for the follow up, support and development of an informed plan of care (utilising Dialogue +) for our Service Users who are under the care of the Community Mental Health Teams and who have accessed A& E Services.
Main duties of the job
The role will be to do preventative work to reduce A& E attendance and to support with contingency plans, signposting, education around what other Services are available and can be accessed as an alternative to A& E and build better links with their CMHT's.

The post holder:
- Will act as an independent practitioner within a defined community setting, while liaising with other professionals as necessary.Will manage a defined caseload and provide supervision to others as appropriate.
- Will be responsible for the development of goal based care plans utlising Dialog+ and have a working knowledge of local, national policies and legislation, which govern current service provision and audit.
- Will be responsible for ensuring effective communication pathways exist.
- Will be responsible for ensuring the promotion of equality diversity and rights in accordance with good practice and legislation.
- Will be responsible for identifying and contributing to the training needs necessary in providing quality evidence based care as appropriate.
