The role is based within Homerton Healthcare providing support to the patients, carers and their families for patients within City and Hackney with haemoglobin disorders.
The post holder will provide services to patients at the acute and community site. The post holder will be responsible for delivering psychological assessment, consultation, formulation, and therapy treatment. The post holder will offer advice and consultation on patients' psychological care to colleagues from other disciplines, working autonomously within professional guidelines and the overall framework of the team's policies and procedures.
To utilise research skills for audit, policy and service development and research within the area served by the team/service. The work will often entail rapid assessment, formulation, consultation, and treatment, jointly with the Sickle Cell & Thalassaemia MDT to enable progress with patient outcomes as well as pain management and reducing frequent attendance at hospital.
Main duties of the job
Where appropriate the post holder will act as an expert in their field and lead and/or participate in initiatives to improve standards of care for this patient group including research for service improvement and promoting evidence-based practice.
The practitioner will practice autonomously, and they will act as an advocate for patients within a multi-professional team setting across various health care sites in line with service requirements.
