Community Forensic Learning Disability/Psychiatric Nurse-

The Post holder will work in the Oxleas FIND ( Forensic Intellectual Neurodevelopmental Disabilities Team),within the Forensic Directorate in providing care coordination and support to individuals with learning disabilities and/or Autism.

The job includes providing intensively supported placements for men and women with a longer than average length of inpatient stay to gain and develop the skills necessary to live in the community. Focus will be on vocational and occupational skills, with social inclusion through a combination of support, access to community facilities, work experience, training, and employment support. There is an emphasis on service user involvement and peer worker support.

Main duties of the job

The post holder will be expected to:

o Provide specialist knowledge and care coordination, including social supervision, through active management and follow up of a defined caseload as well as providing expert advice and consultation e.g. CMHT's, Community LD teams, Community Forensic mental health teams, courts, police liaison meetings, probation and mental health hostels.o Assess, develop and implement care packages for patients within the framework of the CPA in conjunction with other members of the MDT. Works within codes of practice and professional guidelines and is accountable for own professional practice.o Ensure observance and the working of all relevant legal and statutory requirements.o To regularly provide mentorship and supervision to students on placemento To administer risk management strategies including the administration of depot medication in the clients home. Organisation and delivery of prescribed medication to the client for the purposes of close monitoring of medication concordance. Random urine screening for illicit substance misuse as directed by Home Office directions.

Job responsibilities

• To manage a defined caseload of complex and challenging clients who present as high risk and will have demonstrated this through the nature and gravity of their offending.
• To manage the conflict between the therapeutic alliance whilst keeping within the statutory and legal framework.
• To be accountable for the assessment of care needs, risk assessment, development, implementation and evaluation of treatment packages and ensure that reviews take place within the Trust CPA policy.
• Provide care co ordination and / or joint working for a designated caseload of community clients who have committed serious offences and may be subject to formal statutory supervision e.g. conditionally discharged by the Home Office.
• Develop and implement evidence based interventions in the management of MDO's in the community including acting in the capacity as social supervisor and care co ordinator.
• Preparation of social circumstances reports including Mental Health Review Tribunal reports as requested.
