Are you a skilled and experienced healthcare professional with a passion for providing rapid care to those in need within the community? Join our dedicated Urgent Community Response (UCR) team and make a difference in the lives of patients who require urgent care, helping them avoid hospital admissions and supporting them in the comfort of their own homes. Our UCR team also supports the delivery of our Virtual Wards (Hospital@Home) service that facilitates early supported discharge from hospital as well as ongoing admission avoidance clinical management.

Main duties of the job

As a Band 7 AHP Urgent Community Response Clinical Lead (occupational therapist/physiotherapist, you will play a crucial role in assessing, coordinating, and delivering high-quality, patient-centred care to individuals experiencing acute episodes of illness. You will lead a multidisciplinary team to respond swiftly to urgent care needs, providing short-term interventions, and working closely with GPs, hospitals, and other community services.

Key Responsibilities:
  • Lead and support a team of healthcare professionals to deliver timely and effective urgent care services.
  • Undertake advanced clinical assessments to identify and manage patients' urgent health needs.
  • Develop care plans to support patients' recovery and prevent unnecessary hospital admissions.
  • Work collaboratively with other services and agencies to ensure seamless, integrated care.
  • Provide clinical leadership, mentoring, and support to junior staff and colleagues.
  • Ensure the highest standards of patient safety, dignity, and respect at all times.

Person Specification

Qualifications

Essential

  • Degree in relevant Healthcare Profession
  • HCPC registration/NMC registration
  • Teaching and Assessing qualification
  • Evidence of continuing professional development in a relevant area of specialist practice to Masters equivalent level
  • Non medical independent prescriber / supplementary prescribing, or willingness to undertake training within 12mths of coming into post


Desirable

  • Advanced Physical Assessment (level 6/7)
  • Trained to interpret microbiology and pathology results
  • IRMER trained
  • Relevant Masters level degree


Experience

Essential

  • Experience of working as an autonomous practitioner in either an acute or community environment
  • Good understanding of working in partnership with statutory and non-statutory agencies to support patient-centred care
  • Multi-disciplinary and multi-agency working
  • Proven extensive experience of working with adults/older people


Desirable

  • Experience in a Community setting, or gateway area
  • Experience of working between other health and social care organisations /settings


Skills

Essential

  • Able to demonstrate effective leadership and organisational skills
  • Advanced clinical decision making skills to support patient centred care
  • Advanced clinical skills i.e. cannulation, administration of IV medication
  • Advanced physical assessment skills


Desirable

  • Knowledge relating to long-term conditions
