An exciting opportunity has arisen for a mutlidisciplinary Advanced Clinical Practitioner (ACP) (NMC/HCPC/GPhC) at Band 8a to join our Urgent Community Response Services Directorate within the Primary Care, Community, and Therapies Clinical Group.
You will be supported by our Lead ACP, who works closely with the Director of Advanced Practice for the organisation, ensuring appropriate supervision, governance and fulfilling all four pillars of ACP. As part of this dynamic team, you'll collaborate with ACPs, trainee ACPs (tACPs), enhanced practitioners, community matrons, and case managers. About us
Sandwell and West Birmingham NHS Trust (SWBT) is an integrated care organisation with a budget of approximately £600 million and over 7,000 staff.
