An exciting opportunity has arisen for a B6 OT at Wharfedale Recovery Hub. Wharfedale Recovery Hub provides rehabilitation for people who do not need to go into or remain in hospital but cannot be cared for safely at home.
We are looking for a dynamic and enthusiastic Occupational Therapist who will provide person-centred rehabilitation to adults within the recovery hub before going home. Our varied interventions include managing long-term conditions, preventing falls, and facilitating hospital discharges, allowing you to develop a wide and varied skillset.
You will work holistically, using health coaching and self-management approaches whilst working towards the patients goals. You will work as a team drawing on each others strengths and delegate or signpost to other professionals as required.
Main duties of the job
You will work to deliver and facilitate rehabilitation to enable patients to manage their own goals, maximizing opportunities to live a fulfilling life.
You will work closely with patients and their families in the assessment, planning, implementation and evaluation of programmes of treatment and/or care by using person-centred techniques to promote a culture of involvement and empowerment. You will aim to improve recovery and increase independence as much as possible by working as part of a multidisciplinary team to achieve the patients goals.
This job role is for 22.5 hours per week.
