We have an exciting opportunity for a Band 7 Clinical Team Lead to join our Integrated Community Stroke Service Team based at Williton Community Hospital in Somerset. Our teams provide high quality stroke specialist support across the county. To facilitate a robust county wide approach, the successful applicant will work alongside the clinical team lead for the East, and our clinical specialist team. They will be supported by the stroke clinical service manager and consultant therapist for stroke to achieve optimisation of service delivery.

The post holder will provide leadership for the integrated rehabilitation pathways, supporting our multidisciplinary team who provide a wide range of rehabilitation needs. Both within the stroke recovery unit and the community Integrated stroke caseload service.

Our well-established multidisciplinary stroke team consists of Occupational Therapists, Physiotherapists, Nurses, Speech and Language Therapists, Psychologists and Rehabilitation assistants who collaborate seamlessly to support patients along the stroke pathway, making a meaningful difference in their lives. The successful candidate will have experience of working with patients, their families and carers in a range of settings. They will possess exceptional communication skills and be motivated to drive change in diverse environments.

The post is for 30 hours per week with exact working pattern to be established at the time of appointment.

Main duties of the job

The post holder will be responsible for the day-to-day operational leadership and management of Stroke teams, providing services to patients in accordance with agreed objectives, targets, quality standards, controls, and resource constraints alongside the team lead for the east and in collaboration with the clinical specialists.

*Ensure a high standard of care and treatment for patients and to work with other members of the team and other agencies to best meet the patients' health and social needs.

*Play an active role in the development of services in line with the Trust's Strategy and Business Plan.

*Provide clinical support and supervision to staff within the team and to provide clinical leadership.

*will maintain a clinical caseload of 30% of contractual hours, for which they are personally accountable and professionally responsible

*To deputise for the integrated community stroke service manager as necessary.

*To be responsible for recruitment, appointment and supervision of staff.

*To take delegated responsibility for the day to day management of the team budget.

*To monitor expenditure in order to maintain financial balance. To be an authorised signatory for the team budget.

*To manage staff attendance at training, performance, setting objectives and clear expectations of staff behaviour.

*To manage and support staff through change using the appropriate human resource processes.

*To promote a culture whereby complaints are managed in an open and constructive manner in line with Trust Policy. To maintain excellent links with PALS and complaints department staff.

*To support the clinical delivery of patient care.
