Are you a Mental Health Nurse (RMN), Occupational Therapist or Social Worker looking for a new challenge in Assertive Outreach?
Serious mental health conditions can affect someone physically, psychologically and socially. We work across the Bridgwater area to provide specialist mental health services for working age adults living with severe, complex or enduring mental health conditions such as acute depression, psychosis, complex trauma or anxiety.
You will work as part of a team, providing care and support to an allocated group of services users with complex needs, across a shared team caseload. You will support service users in the community and also those who are in-patients in supporting their return home and to promote recovery. You will assess, plan, implement and evaluate care plans of the service users we work with.
This opportunity is for 3 days per week (22.5 hours), 9am-5pm - we can be flexible around days, condensed hours and start / finish times.
We are a supportive, compassionate multi-disciplinary service where every person is an integral part of the team.
Working as part of the Community Mental Health team, is one of the most rewarding roles I have undertaken. Every day is different -- it is interesting, challenging and I love being able to make a difference. From the moment I started, I felt relaxed and included.
As part of our Community Mental Health Team, you can take part (or not) in team days/nights out, sweep stakes and 'secret Santa', as well as other activities.
