We have an exciting opportunity to recruit a Band 6 Clinical Nurse Specialist in the Asthma Service. This is for a one-year fixed term contract.

The role will involve supporting the asthma service and will have a focus on introducing an app-based tool to support patients with asthma management.

The post holder will support the wider asthma service in a variety of ways including, providing rapid access clinics, case finding, pre-assessment telephone clinics, face to face clinics, community link and liaison between primary and secondary care. There will be an educational component to the role, primarily for internal staff but also for our local community referrers. There will also be audit and research opportunities.

The successful candidate will have some experience in asthma, either in a ward, community or out-patient setting. They will have at least two years post registration experience. They must be able to practice autonomously and be self-directed, but also be able to operate efficiently as part of a team. They will demonstrate an eagerness to learn and develop and have a passion for education. They will have the opportunity to contribute to the development of the service.

Main duties of the job

The postholder will work closely with the lead clinician, Dr Roy and with a Band 7 Asthma Nurse Specialist. They will be supported by the Lead Respiratory Nurse Specialist. They will also be part of the wider Respiratory team at UCLH. This role will provide an excellent opportunity to develop leadership skills in relation to change management and service development.

1. Case finding within the trust, and the community via MDT's and diagnostic hubs.

2. Support asthma management, by reviewing patients using the digital asthma app, and other clinical information gathered in order to triage referrals effectively.

3.Pre-clinic assessment to support one-stop clinics and straight to test opportunities4. Deliver patient centered care by acting as a point of contact for patients and GP's by ensuring there is a community link with primary care services.5. Supporting patient initiated follow up (PIFU): educating patients when to self-manage and when to trigger PIFU.6. Education and training. To support the educational promotion of strategies for the long term and acute management plans associated with asthma across the community and the trust.7. Undertake patient assessment using a range of different methods, which may include physical examination, ordering and interpreting diagnostic tests or advanced health needs assessment.8. Draw on a diverse range of knowledge in decision-making to determine evidence-based therapeutic interventions.

• Registered Nurse Part 1, 2 or relevant section of the register
• Relevant post-registration programme

• Completed/completing Master's Degree or modules at MSc level
