We have an exciting opportunity for a well-motivated, experienced Occupational Therapist to join the First Response Therapies (FRT) Team in a static position following new investment in the team.

The post is based at Aintree University Hospital involving acute assessment and management of patients within the urgent care setting (Emergency Department, Frailty, Acute Medical and Surgical Assessment areas) preventing deconditioning, supporting rapid hospital discharge and preventing unnecessary hospital admissions. The role includes community based activity for at risk patients utilising a 'discharge to assess' model of care.We aim to work closely with the Multi disciplinary Team working at the front door of the acute hospital to enhance frailty services and and the ambulatory care model.

The role requires excellent prioritisation, organisational and communication skills alongside effective teamworking. An ability to demonstrate assertion and creativity with assessment and treatment planning and methods of service delivery are essential in this dynamic, fast-paced environment.

This role requires flexibility working across a 7 day roster pattern, 8am-8pm. Previous clinical experience within an urgent care setting is desirable.

Main duties of the job

• Physical and functional assessment, treatment and discharge planning for patients with routine and complex medical, musculoskeletal, frailty and neurological conditions.
• Assessment and provision of appropriate aids/adaptations to promote functional independence.
• Comprehensive cognitive assessment and treatment for patients with acute/chronic cognitive impairments.
• Undertaking community based environmental/follow up visits enabling rapid hospital discharge and reducing hospital readmission risk.
• Collaborative working with FRT Physiotherapists, Assistant Practitioners, Therapy Assistants and the wider MDT.
• Participate in the clinical coordination role for the team.
• Support training and development of staff, delivering formal/informal training.
• Clinically and pastorally supervise therapy staff and University students.
• Participation in audit/service delivery to enhance patient care.
• Support Band 7 therapists in team performance monitoring and management

Clinical

To assess and identify the individual need of referred patients and their carers, planning and implementing appropriate treatment programmes within both hospital and community settings.

In accordance with best practice, implement and evaluate Occupational Therapy Intervention.

To record and report any findings appropriately following the standard procedures used by the department, including any electronic based systems and maintain high levels of written/verbal communication.

To share skills/knowledge with other disciplines and patient/carers

To liaise with hospital/community based staff as appropriate to ensure continuity of treatment and provision of equipment/adaptations to facilitate hospital discharge, prevent re-admission and support resettlement/re-enablement into the community

To assist with the development and provision of areas of specialist practice within OT that are relevant to local clinical and service need.

To maintain awareness and regularly review information, resources regarding community services and agencies to compliment practice, advising and referring on patients as appropriate.

To work collaboratively with the MDT providing information and resources in support of patient care.

To provide OT intervention to service users which supports equality and values diversity.

Managerial/Administration

To be aware of and identify changes in service demand and report to senior staff.

Contribute to the ordering and maintenance of materials/equipment/resources to facilitate patient care.

In the absence of senior staff to take responsibility for the daily running of the department

To attend/participate in Therapy briefing sessions and to feedback and inform others as appropriate.

To be actively involved in daily management and allocation of workload and any issues arising in relation to the location of cases.

To record and monitor statistics and data regarding referrals and interventions.

Qualifications

Essential

• Diploma / Degree in Occupational Therapy or equivalent
• HCPC Registration

• Member of Special Interest group

Experience

Essential

• Post registration experience in a junior rotational post providing a broad experience in musculoskeletal / medical surgical conditions
• Evidence of a contribution to clinical education of less experienced staff, students or assistants
• Evidence of participation in audit
• Evidence of the advanced skills required to plan complex and rapid discharges

Knowledge

Essential

• Evidence of sound clinical knowledge and specialist skills relevant to the clinical caseload
• Broad knowledge of current best practice in Occupational Therapy
• Have an understanding of the legal responsibilities of the profession

Skills

• Ability to manage own work load, determine priorities and delegate tasks
• Effective written and oral communication skills
