Hywel Dda University Health Board presents an exciting opportunity for a motivated and experienced Occupational Therapist to join our NHS & Public Health Award Winning Intermediate Care Team.

Hywel Dda University Health Board - Introduction of an Intermediate Care MDT, Carmarthenshire - Public Health Wales (nhs.wales)

We are an innovative, comprehensive team of Occupational Therapists, Physiotherapists, Therapy Assistant Practitioners, Dietitians, Nurses, GP's, Advanced Paramedic Practitioners, Advanced Nurse Practitioners, Physician Associates, Social Workers, Senior Reablement Workers, and Third Sector Agents striving to deliver the right support, at the right time in the right place as close to home as possible.

These roles will form part of the Home First Multidisciplinary Team (MDT), all of whom work together to provide early intervention, period of rehabilitation and assessment in the community to immediately mitigate loss in function, risk of deconditioning, nutritional deficit, deterioration in their mental health to increase people's time spent at home rather than hospital.

Our approach is to deliver patient care in a holistic, person-centred manner, supporting patients, when possible, to stay/return safely to their own homes where they may continue to be a valued member of their own community, within which they are most familiar and settled.

Main duties of the job

The post holders will input into our home based and bed based intermediate care services here in Carmarthenshire.

As a versatile postgraduate, you will need to have a strong clinical grounding in older people rehabilitation, complex manual handling, expertise in frailty syndrome, neurological conditions, cardio respiratory conditions, a good understanding of current issues affecting intermediate care and community services.

Successful applicants will require an excellent ability to work autonomously, managing and prioritising a diverse and unpredictable caseload, with the ability to adapt to a rapidly changing and pressurised environment alongside members of the MDT. Post holders will be expected to support and play a role in supervising junior Occupational Therapy staff, OT Technicians, Rehabilitation Assistants/Frailty Support Workers, and students.

The post holder will be required to participate in the provision of a flexible 7 day service, based on service need. This will be on a rota basis.

Person Specification

Knowledge

Essential

  • Knowledge of relevant current health and social care legislation applicable to role, including: Social services and wellbeing (Wales) act 2014, Childrens Act 1989, manual handling operations regulations 1992 and the housing and regeneration act 1996, The Future generations act (Wales) 2015 and A healthier Wales , 2018,
  • Extensive knowledge of daily living and manual handling equipment, adaptations and design features which can assist service users and their carers in maximising independence
  • Knowledge of national and local resources that would benefit service users
  • Knowledge and application of Microsoft Office software.
  • Knowledge of a range of rehabilitation interventions across all therapies.
  • Understanding and application of clinical governance, including research and audit methodology.
  • Understanding and application of relevant outcome measures to demonstrate effectiveness
  • Understanding of the roles of other staff groups in health and social care.


Desirable

  • Knowledge of other services that link across primary/secondary/intermediate care
  • Working knowledge of therapeutic equipment
  • Knowledge of Integrated service priorities


Qualifications, Vocational training and Professional Memberships

Essential

  • Degree in Occupational Therapy (or equivalent Diploma).
  • Current HCPC registration.


Desirable

  • Accredited Practice Educator
  • Accredited Agored Assessor


Experience

Essential

  • Significant post graduate experience of working as an occupational therapist across a range of settings and variety of conditions.
  • Working with service users that are vulnerable, have a physical disability or an older person
  • Previous experience of working in a Health, Social services or housing organisation.


Desirable

  • Experience of working with groups
  • Experience of working in a community setting
  • Experience of using virtual communication platforms for delivering service
  • Experience in same clinical area as post being applied for
