An opportunity has arisen for a Higher Level Pharmacy Technician to join our Integrated Community Services (ICSD) Pharmacy team. The ICSD Pharmacy team plays a significant role in supporting patients following an acute hospital discharge or to prevent avoidable admissions to hospital during periods of acute illness.

The role will include working as part of a multidisciplinary team.

The objective of the service is to offer patients the best opportunity to maximise their potential for regaining independence, and therefore relies on an integrated and committed multidisciplinary team to deliver the shared goal.

The successful candidate will work closely with patients, relatives and carers, to help optimise, individualised medication regimens. Our goal is to support patients to reach an appropriate level of independence with their medicines and to support other healthcare professionals gain insight into medicines management.

The ideal candidate will be a highly motivated individual with a willingness to work independently and with other health care professionals at all levels.

The successful applicant will be undertaking CQC regulated activities and are encouraged to be fully vaccinated against COVID-19 to protect patients.

Main duties of the job

Work with other members of the ICSD Pharmacy team to improve medicines management across bed and home based community services.

To gather information concerning patients' current medication/medical history and to liaise with other service providersto improve medicines management in the community.

Support safe and cost effective delivery of pharmaceutical services.

Actively participate in the development and implementation of systems and/or services to support patients with their medicines

Carry out domiciliary visits to patients where appropriate.

Provide basic pharmaceutical advice to patients, families & carers, other services.

Provide assessment of patients ability to self medicate.

Support patients/carers in the administration of medicines.

Participate in team meetings/MDT meetings as may be appropriate.

Carry out clinical audit collection, prepare written reports of audit outcomes.

Establish effective working relationships with other community based teams, secondary care, community pharmacists, CCG medicines management team, GP practices, primary care networks and social services to ensure medicines management issues are communicated and dealt with effectively.

Person Specification

QUALIFICATIONS

Essential

  • NVQ Level 3 with BTEC in Pharmacy Services with appropriate underpinning knowledge or equivalent
  • Registered as a Pharmacy Technician with the General Pharmaceutical Council (GPhC)


Desirable

  • Evidence of further education in clinical pharmacy practice
  • Good all round technical knowledge and basic clinical knowledge


EXPERIENCE

Essential

  • Ability to work under own initiative and with supervision
  • Prior experience of work in a hospital pharmacy, community pharmacy, PCN or CCG
  • Able to meet deadlines whilst paying attention to detail
  • Satisfactory employment history
  • Ability to cope with occasional distressing circumstances e.g. when dealing with seriously ill patients
