An exciting opportunity has arisen for an enthusiastic, well-motivated and flexible NMC registered adult nurse to join our Recovery at Home Team in South Tyneside.The team is multidisciplinary consisting of Nurse Practitioners, OTs, Physiotherapists, Healthcare Assistants, Therapy Assistant/Exercise Practitioners and administrative staff. The team also works alongside partner agencies including those from the local and voluntary sector.

This role involves working over a 24-hour period, 7 days a week, delivering unplanned care to people in their own homes, to assist in preventing hospital admission or supporting those who have recently been discharged from hospital and require enhanced intervention and short-term support.

The service is fast paced, and you would be expected to see a wide range of patients who present with a variety of conditions.

This is a unique and rewarding role which will allow you to work with patients within their usual place of residence to enable them to remain there safely alongside you developing your own skills.

Manual Car driver essential we have access to pool cars for completion of patient visits.

Main duties of the job

The service covers 24 hours, 7 days per week, it is fast paced, and you would have the opportunity to see a wide range of patients who present with a variety of conditions.

Job description

Job responsibilities

  • To be responsible and accountable for the delivery of safe, effective and compassionate nursing care for patients designated to their care.
  • To provide care to people in their own homes to prevent admission / re-admission to hospital, by providing care to patients in the community over a 24 hour period.
  • Undertake comprehensive holistic assessment of patients in both planned and unplanned situations, be able to prioritise clinical need, initiating investigations, determine a differential diagnosis within scope of practice and implement a plan of care reflecting evidence based guidance.
  • To always work to the NMC Code.
  • To supervise non-registered staff as stipulated by the NMC under the requirements of delegation.
  • Provide effective leadership and assist with the management and organisation of the team in the absence of a senior nurse.
  • To comply with the Trusts vision, values and behavioural compact.

Person Specification

Qualifications

Essential

  • Current NMC Registration (Level 1 adult nurse) or evidence of qualification if newly qualified
  • Diploma in health related study or equivalent appropriate qualification


Desirable

  • Degree in nursing studies or healthcare related subject (UK equivalent)
  • Mentorship certificate/Practice Assessor training or demonstrable equivalent experience


Skills and Knowledge

Essential

  • Ability to provide and receive complex, sensitive or contentious information
  • Able to work without supervision
  • Competent in the use of IT such as Microsoft Office
  • Evidence of CPD
  • Knowledge of national health and social care policy
  • Knowledge of evidence based guidelines e.g. NICE guidelines, NPSA alerts, current nursing legislation


Desirable

  • Venepuncture and cannulation competency


Experience

Essential

  • Experience of working in an acute hospital or community healthcare setting


Desirable

  • Recent experience of working as a registered nurse


Physical Skills

Essential

  • Ability to assist patients to move as per Trust Moving and Handling Policy
  • Car Driver (Manual)
