You will be part of The Mid Beds Community Mental Health Services. Our team is part of the Mental Health Transformation of services and our new blended team where our voluntary service colleagues are fully embedded within the team. Main duties of the job

You will work as an autonomous practitioner within a dedicated Multidisciplinary Hub. If you are committed, relish new challenges and strives for excellence, this post is for you.

You will hold a caseload and work within the principles of the agreed model of care for the service as laid down in the Operational Policy.

This will involve the assessment, care planning, implementation and evaluation of the care provided and will liaise with all agencies involved with the clients. The role includes being part of a duty rota, screening referrals and participating in Quality Improvement (QI) projects within the Team.

You will also take part in the Transformation Agenda in line with The NHS Long Term Plan, which describes:

New community-based offer which will include access to psychological therapies, improved physical health care, employment support, personalised and trauma-informed care, medicines management and support for self-harm and co-existing substance use and proactive work to address racial disparities.

  • Comprehensive needs assessment and care planning ensuring this is holistic, person centred, co-produced utilisingDIALOG+
  • Specialist clinical assessments and care delivery in line withbest and evidence based practice with reference to NICErecommendations
  • Case management
  • Crisis intervention
  • Safeguarding awareness, alert raising and acting asenquiry officer
  • Awareness and implementation of MHA and relevant legalframeworks. Good report writing and presentation of case
  • Trauma informed practice
  • Carers support and Psychoeducation
  • Liaison and interface with other agencies
  • Ensuring that a service user centred multi-disciplinaryapproach to care is present
  • To take an active role in direct service user care and act as arole model at all times
  • Demonstrate an understanding of the Care Act associatedresponsibilities within your role
  • Ensuring that care reflects individual and cultural needstaking into account any reasonable adjustments that may berequired
  • Understand and apply the aims and principles of healthpromotion, protection and improvement and the prevention ofill health when engaging with people
  • Undertake training and assessment to expand clinical skillsand knowledge relevant to clinical practice needs.
  • Ensure that the clinical environment is pleasant, clean,comfortable and suitable for use at all times liaising with otherdepartments as necessary. Safe clinical management of theclinic room and equipment


Person Specification

Education/ Qualificatio n/ Training

Essential

  • Relevant professional qualification
  • Has obtained MSPP qualification or equivalent


Desirable

  • Evidence of further training, e.g. Bsc Community Mental Health Nursing or AMHP status or Clinical skill based qualification e.g. CBT, Psycho-social interventions Non-medical prescribing.
  • Degree module in Psychopharmacology
  • Degree module in physical health


Experience

Essential

  • Post qualification Experience


Desirable

  • Experience of working in a community mental health team

  • Good understanding of mental health conditions and treatment options in line with NICE guidance
  • Physical health assessment and monitoring
  • Administering of depot medication

  • Risk assessment relapse prevention health promotion and prevention/education establishing good relationships with good communication skills with SU's/carers/MDT/partner organisations chairing meetings presenting cases comprehensive report writing managing clinic inclusive of depot and physical health psycho-social intervention skills
