The Children's Therapies service provides Occupational Therapy, Physiotherapy, Speech and Language Therapy,Specialist Nurse Advisor and Care Coordination to children aged 0-19 within Kent Community NHS Foundation Trust(KCHFT).Clinicians are responsible for managing a paediatric clinical caseload in the locality.The Children's Therapies Services provide a high quality and cost effective clinical service to children/young people andtheir families. In order to do this, clinicians are required to:- Work with key colleagues (ie education and health) in order to support multi agency models of service delivery.- Participate in reflective practice and on-going learning in order to continually improve the service offered by Children'sTherapies.- Be committed to continually improving the patient experience by engaging in regular patient feedback mechanisms andencouraging service users to participate in shaping the service delivery.- Work to the standards defined by the service clinical competenciesCare co-ordination is a service for babies and children (up to the age of 12) who present with an identified developingcondition or disability which is impacting on, or has the potential to significantly impact upon their long-term developmentand / or function.
Main duties of the job
- Co-ordination of care with other professionals and organisations.- To provide information on local groups and other agencies- To support with transition into nursery and/or school- To run/support team around the child meetings to share information with other professionals involved in the child'scare and put together a family plan- To support referrals onto other agencies /services.- To work with nurseries/schools, GPs, health visitors, doctors, preschools, and other health professionals. They will link inwith other agencies involved in the child's development.- To participate in Multi disciplinary assessments with the therapy colleagues- To support supervision and development of staff- Acting as lead and senior practitioner to support junior staff carry out assessment/reassessments of service usersand/or pathways with complex needs across the relevant field.- Developing integrated care programmes in partnership with senior practitioners and/or commissioners.- Supporting and making recommendations to senior managers in order to prioritise and focus efforts to drive forward and achieve business objectives, service improvement strategies and statutory requirements.
