We are looking for enthusiastic and dynamic Advanced Nurse Practitioners to join the new innovative specialist Hospital@Home Frailty Team/ Frailty Assessment Unit. We are in the process of developing a new pathway which involves the identification and management of frailty patients within their own homes and across the acute site.

The ANP will use expert knowledge and advanced clinical reasoning skills to provide specialist support and a comprehensive geriatric assessment to aid decision making and plans for ongoing management. The ANP will support admission avoidance and support the Hospital@Home model.

The post holder will be part of a larger multi-disciplinary team including Consultant Geriatricians, ANPs and Therapists. This post would suit an experienced nurse who has an understanding of the management of people living with frailty.

This is a rotational across frailty services.

Main duties of the job

The successful candidate will provide leadership, clinical expertise and organisation of the new Frailty H@H Service.

They will continuously develop and enhance knowledge and skills that will directly influence consistent standards of clinical practice and ensure the highest possible standards of evidence based care are achieved in order to promote patient focused care.

They will promote patient centred high quality evidence based care to patients on the ward, ensuring the patient is central to all aspects of treatment and care.

Communicate effectively with all the multi- disciplinary teams both internal and external to the organisation.

Provide clinical leadership to a group of staff within the H@H team and throughout the care group.

Job responsibilities

The Frailty Advanced Nurse Practitioner will be experienced in the management of frail patients supporting admission avoidance and safe, prompt return to the community.

You will have expert knowledge and advanced clinical reasoning skills to provide specialist support and advice to the Emergency Department team and other healthcare staff and to manage a case load of patients within the Frailty Assessment Unit where the length of stay will be less than 72hrs.

Assess, diagnose, treat and discharge adult patients ensuring appropriate advice, health education and referral to appropriate community services.

To perform autonomous diagnostic and prognostic investigations

To make autonomous decisions regarding admitting or discharging of patients within ED or the Frailty Assessment Unit using advanced clinical knowledge to underpin decisions.

Work independently, with a high degree of autonomy, and take part in clinical supervision from a named consultant.

Be able to manage your own caseload in response to unpredictable patterns of work. Accept referrals from other health care professionals and work unsupervised.

To clinically assess patients, prioritising clinical need and highlighting those patients who require immediate medical intervention.

To act as a resource for all members of the multi-disciplinary team, providing advice concerning aspects of the management of frailty.

Support the ongoing development of the Frailty Team and Unit with an integrated approach to the delivery of acute medicine and unscheduled care involving all relevant stakeholders including health and social care organisations.

The post requires a flexible approach to working and requires the post holder to work 8am-8pm covering a 7 day service.
