Chapeltown Primary Care Network (PCN) have an exciting opportunity for a full or part time Clinical Pharmacist working within the pharmacy team as well as the wider PCN and practice teams at Allerton and Westfield Medical Centre, St Martins Practice and Woodhouse Medical Practice.
Chapeltown is a friendly and dynamic network caring for a diverse population with a wide range of complex health needs, supporting the management of long term conditions, while striving to tackle health inequalities.
The successful candidate should be ambitious and motivated, with a focus on patient care.
Main duties of the job
Working as an integral member of the pharmacy team the successful candidate will undertake a variety of tasks in both clinical and non clinical settings, ensuring the achievement of key deliverables and defined targets.
The Clinical Pharmacist will be patient facing and responsible for Structured Medication Reviews, specialists clinics as well as day to day practice tasks.
