Southampton Neonatal Services are looking to recruit an passionate RN / RM with experience in neonatal nursing to join our Home Team as a band 5 Neonatal Outreach Staff Nurse.

Southampton Neonatal Services care for patients and their families across the South Coast region including the channel islands, providing intensive care, high care and special care, as well as having a Transitional Care Unit and a community outreach team to support discharge planning and care in the home. We host the regional transfer service, SONeT, ensuring that families receive care in the most appropriate hospital, closest to their home. We are a tertiary referral centre able to offer a range of specialities to our patients across the Thames Valley and Wessex region, including neonatal surgery and cardiac services.

Job description

Job responsibilities

Working within the established Neonatal Home Team, you will participate in assessing, planning, implementing, and evaluating individualised discharge plans for our neonatal and transitional care patients and their families. Working with the multidisciplinary team, you will anticipate discharges, co-ordinate arrangements and follow up plans. You will support families with the transition from hospital to home, providing education and emotional support as necessary. You will continue to provide holistic family-centred care to patients in the home environment in partnership with their families, whilst supporting junior staff to do the same.

Person Specification

Qualifications / training required

Essential

  • Registered Nurse (Adult or Child) on Part 1 of the NMC register or Registered Midwife
  • Evidence of ongoing personal development
  • Neonatal Life Support qualification


Desirable

  • Recognised post qualifying module in neonatal care
  • Mentorship (or equivalent) module


Previous or relevant experience necessary

Essential

  • Experience of working on a neonatal unit
  • Understanding of personal accountability
  • Child and adult safeguarding awareness


Desirable

  • Community Experience
  • Further skills experience, e.g. venepuncture


Aptitudes and skills required

Essential

  • Enthusiastic attitude. Good interpersonal skills. Acknowledges strengths and limitations Desire to progress within neonatal care
  • Interest and passion for discharge processes and community care and clear rationale for applying for post
