Main duties of the job

The post holders will work with the Community Nurses and Adults Social care staff to provide a first class service to the residents of Stockport.

We are looking for a flexible, innovative practitioner with a variety of clinical skills. The District Nursing Service provides care in patients' homes as well as residential homes caring for patients with complex needs and supporting carers.

The post holder should have good organisational and assessment skills be flexible and willing to cover the service between 08.00 and 18.00 over a 7 day period.

Job description

Job responsibilities

This post requires good communication and interpersonal skills and the ability to be adaptable, reliable and approachable. In addition you will need to be able to demonstrate up to date knowledge on relevant nursing issues with knowledge of Stockport Together and the integration of services in Stockport. The post holder will need to lone work, and be a car driver and have a car available for work.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.

Person Specification

Education & Qualification

Essential

  • Current 1st level registration with NMC
  • Grade C (or equivalent) in GCSE English


Desirable

  • Diploma in Nursing Practice or degree
  • Evidence of Health related study at level 2/3
  • Teaching and Assessing Certificate


Knowledge

Essential

  • Demonstrate knowledge of current local and national health agenda and developments.
  • Awareness of current clinical issues in out of hospital care


Desirable

  • Demonstrate an understanding of an implication of an issue which could impact on community service provision.


Experience

Essential

  • Delivery of patient care and working in a clinical area
  • Evidence of teaching and assessing students and junior members of staff.


Desirable

  • Evidence of developing area of expertise eg Tissue Viability/ Frailty/ End of Life care
  • Experience of audit involvement in project work


Skills & Abilities

Essential

  • Effective time management
  • Ability to work under pressure

  • Computer literate
  • Demonstrate an awareness of patient dignity and respect.
