We are looking for a Specialist End of Life Community Nurse to join our Community Nursing Team based along the South Kent coast.

Our Community Nursing Teams provide nursing support for patients aged 18 and over in their home, delivering personalised care to those with complex and long term conditions so that patients can stay safe and well at home or in their chosen environment. The service may provide wound management, change wound dressings, palliative and end of life care and symptom control and catheter care.

Main duties of the job

In this role you will be responsible for the supervision, teaching and leadership of individual staff members and the quality of the care they deliver.

The team you work with will be aligned to a Primary Care Network (PCN) where you will represent community nursing and provide secondary linemanagement to support the PCN lead.

You will need to be an experienced Community Nurse with non-medical prescribing experience or awillingness to undertake to provide sound clinical judgement and act as a role model for the profession. Asthe senior clinician you will be a decision-maker and a problem solver with accountability for your team's performance.

Due to the community nature of the role you will need to be able to drive your own car.

Additional Working Pattern: 35.7 hrs- Mon- Friday with flexible weekend working. Service cover 8am-6pm

Qualifications

Essential

  • 1.Registered health care professional (NMC registration) (1). 2.Health related degree and evidence of relevant post graduate development at Level 6/7 (1). 3.Evidence of Post Graduate Specialist qualifications relevant to area of practice (examples: SPDN, Mentorship, Coaching, Clinical Expertise Pathway) (1). 4.Undertaking Non-Medical Prescribing and Advance Clinical Practice Skills (2).
  • Valid driving licence in the UK with access to a vehicle


Desirable

  • 1.Non-Medical Prescribing qualification (1). 2.Advance Clinical Practice Skills and Clinical Reasoning qualification (1). 3.Management or supervisory qualification/ programme of study (1).


Experience

Essential

  • 1.Minimum of five years' experience in area of practice.
