This role of Band 7 Occupational Therapist provides an exciting opportunity for the post holder to lead and provide clinical input, oversight/assurance, development and supervision for the Occupational Therapists working within the Trust's In-Patient Community Beds setting. Work will be focused within the Hazelwood ward at Isebrook Hospital in Wellingborough which is a 32 bedded unit providing in-patient rehabilitation to patients presenting with general rehabilitation needs and also rehabilitation needs post stroke. As such the post holder will be required to have the relevant clinical experience across both clinical fields of rehabilitation and to support other members of the MDT in their development.

To complete initial and ongoing patient assessments, support patients to identify SMART goals, utilise clinical outcome measures and develop, deliver and progress bespoke treatment plans.

- Enable patients to work towards goals relevant to the in-patient community hospital setting
- Support the patient and family members/carers with regards to discharge planning
- Identify further rehabilitation needs to include onward referral to other services as appropriate
- Demonstrate critical thinking in the clinical decision-making process. Evaluate the effectiveness of the treatment through autonomous and accountable decisions in Occupational Therapy practice supported by current research and in accordance with NHFT protocols and policies.
- Provide leadership, education and support to other team members through promotion of innovative strategies to manage change and improve service delivery.
- The role will ensure that flow into and out of the service are maintained and also ensuring a flexible model of service delivery is maintained to meet the needs of the service.
- Responsible for identifying gaps in the service and assisting with service improvement and developing pathways of care to meet the service needs.
- Communicate condition related information, sensitive diagnostic & prognostic information to patients/clients, carers & relatives, requiring empathy & reassurance.

- BSc/MSc in Occupational Therapy
- Active registration with the Health Care Professions Council
- Evidence of CPD and clinical skills relevant to the areas of Stroke and rehabilitation
- Team leadership experience
