An exciting opportunity has arisen to join the paediatric team at Moorfields in a developing CNS role.
As a senior nurse, the CNS is a key member of the specialty and Divisional nursing team with a responsibility to contribute to wider service delivery. However, this role will primarily focus on the delivery of the uveitis service, co-ordination of shared care, disease management and education and MDT discussion of complex or shared care patients, facilitating patient transition between North Central London Trusts to young people's clinic.
This post holder will work Mondays, Tuesdays, and Thursdays at Moorfields to cover all paediatric and young people's clinics and theatre sessions. They will attend GOS on Fridays for the Rheumatology/Uveitis clinic to meet and liaise about patients who will transition to Moorfields and assist with education and MDT discussions for patients sitting across the Moorfields, GOS and UCL pathway. The main responsibilities below are an indication of the role in its fullest sense but development and training will be given to the successful candidate to develop into this role.
Main duties of the job
The Uveitis CNS will be attending all uveitis children's and young peoples (CYP) clinics, they will support CYP and their families with education about their treatments and monitoring of these treatments.
Some specific treatments are given at specific sites , the CNS will gain knowledge of this ensuring that follow-up appointments are on sites where this treatment can take place. They will work closely with consultants, paediatricians and specialist pharmacists and liaise with families regarding blood results and general side effects the CYP may be experiencing.
Part of the role will also be speaking with CYP who are approaching age of transition to young peoples services and beyond and completing trust documents and facilitating and empowering the CYP's autonomy of their eye conditions and how it will be monitored and manged as they move to adult services. This post also has line management responsibilities.

Direct, support, encourage and advise clinical teams, by the provision of specialist knowledgeand advice in the care of patients within the area.
Participate in the promotion of patient centred care, and establish and maintain a supportiverelationship with the patient and their family during the period of assessment, ongoingtreatment.
Support and enable patients and carers to make informed decisions relating to their treatmentand management.
Promote high quality clinical care, and assist in the development of the specialist service,through the use of evidence-based practice and clinical standards
