The Neurology/Motor Neurone Disease Team has an exciting opportunity for an Advanced Nurse Specialist in MND. This post will be based at York Hospital but will have responsibility for providing a cross site service for MND patients, using a home visit model.

The successful candidate will have responsibility for the management of a defined case load, and will lead and shape nursing services for people living with MND. Excellent communication, multi-disciplinary team working and an ability to work autonomously are all essential skills for this role.

This is a highly rewarding but demanding role and it is essential that candidates are extremely motivated, proactive and willing to learn. The successful candidate will benefit from supervisory support from the MND and wider Neurology Team, and will also have opportunities for continuing professional development.

Access to a vehicle that can be used for business purposes and hold a full and valid driving licence (role will involve travel to numerous addresses that cannot be easily accessed by public transport means) is required.

Main duties of the job

o Manage the care of a defined MND caseload using a home visit clinical model

o Provide specialist nursing care and advice to this specific patient group, and contribute to the development of the speciality MND service

o Continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care

o Demonstrate leadership in service improvement and continued professional development

o Liaise with multi-disciplinary colleagues to co-ordinate the complex are needs of MND patients

o Maintain professional links with regional colleagues to share specialist knowledge and service development

Essential

  • Experience at Band 6 level or equivalent within speciality
  • Managing staff
  • Service development and / or change management


Qualifications

Essential

  • Registered Nurse (Level 1)
  • Recognised teaching / assessing qualification
  • Post registration qualification in specialty


Desirable

  • Non medical prescribing qualification dependant on service needs
