Main duties of the job
What You'll Do: As an Urgent Community Response Band 6 Nurse, you'll:
- Provide high-quality, evidence-based care to patients experiencing health crises in their own homes.
- Assess, plan, and deliver complex care interventions, including medication administration and wound care.
- Work collaboratively with multidisciplinary teams to ensure seamless patient care.
- Utilize innovative approaches to improve patient outcomes and efficiency.
- Adhere to strict safety protocols, including lone working guidelines.
What We're Looking For:
- Registered Nurse with a valid NMC PIN
- Strong clinical skills and experience in community or acute care settings
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Passion for delivering exceptional patient care

- Registered Nurse with current UK NMC registration
- Evidence of post-registration education and training
Desirable
- Advanced Clinical Assessment and Examination qualification
- Member of a Special Interest Group relevant to this post
Experience
Essential
- Demonstrate 2 years' experience post-qualification
- Previous experience of undertaking comprehensive health needs assessment and writing care plans
- Demonstrate knowledge of the purpose and value of supervision.
- Experience of teaching of junior staff / students, peers in-service training setting.
- Experience of negotiating and communicating within a multi-disciplinary team
- Experience of teaching, patient and public involvement and health promotion in practice
- Experience of providing acute care response to frail and elderly patients in acute or community setting
- Experience with relevant clinical competencies including urethral and suprapubic catheterisation, palliative and end of life care support, syringe-driver set up and symptom control.
- Experience of working in the NHS
Desirable
- Participation in research, service development or audit projects
- Previous experience working in community setting
- Previous experience in urgent community response or community nursing
