The Cardio-Respiratory CSU has an amazing opportunity to recruit an enthusiastic and motivated Discharge Planning and Patient flow specialist nurse team leader to lead the patient flow and discharge team. You will be working closely with the CSU Multidisciplinary team, CSU management team, Winter room, Corporate discharge team, Corporate Operational team and community partners across organisations to provide safe, appropriate and timely admission and discharge from LTHT

We are looking for a team leader who would value the opportunity to be part of a dynamic developing service working with health and social care partners to facilitate patients to maximize their opportunity for independence on discharge from hospital. You will gain experience strategically supporting the Cardio-Respiratory CSU Matrons and the Head of Nursing to improve patient pathways across LTHT, LCH neighbourhood teams and the community rehabilitation teams

Job responsibilities

The Jobholder will carry continuing responsibility for a defined caseload, including the assessment of care needs and the development, implementation and evaluation of programmes of care. The jobholder will provide highly specialised nursing advise specific to the patient group or speciality.

The job holder will contribute to the development of the speciality service as well as contributing to the strategic direction of nursing within the Trust.

The jobholder will manage and lead a team of nurse specialists at Band 6 and Band 4 below and be responsible for the effective use of resources within the clinical team.

Experience

Essential

  • Experience at Sister/Charge Nurse level or equivalent within speciality
  • Audit & Standard setting
  • Managing Staff
  • Current issues in healthcare


Desirable

  • Service development and/or change management


Skills & behaviours

  • Capable of lateral thinking
  • Excellent written and verbal skills
  • Ability to organise and prioritise own and others workload


Qualifications

Essential

  • Registered Nurse (Level 1)
  • Recognised teaching/assessing qualification
  • Post registration qualification in specialty
  • Evidence of continued professional development
  • Willing to undergo training as necessary


Desirable

  • Extended/ Supplementary Prescribing
