Are you an experienced, motived and enthusiastic physiotherapist looking for a new challenge? We are seeking a confident, autonomous practitioner with high level communication skills and a flexible, problem solving, holistic approach to care.
The service is a Multidisciplinary Rehabilitation Service that is specialist in Neurological Conditions. It is community-based and community-focussed, providing a specialist rehabilitation service to the person in the place most suited to their current need, which includes, but is not limited to, their place of residence, day centre, place of work.
As the successful candidate, you will join a forward looking and supportive team, who actively support CPD and service development. The post holder should have proven experience of working with adults with a wide range of neurological conditions, including complex cases in a multi-disciplinary team.

Main duties of the job
To provide specialist physiotherapy input to clients as part of the specialist Community Neurorehabilitation service.
To provide specialist neurology physiotherapy assessment and treatment.
To work with specialist nurses and other health and social care practitioners to manage those with neurology long term conditions.
To maintain competency in manually handling people and therapeutic manual handling.
To identify possible areas of development for the physiotherapy service in the Specialist Community Neurorehabilitation team, and work with senior therapists and team lead in developing them.
To keep abreast of evidence based and best practice.
To ensure good working knowledge of national and local standards, and monitor quality through participating in clinical audit and evaluation work as required.

Job responsibilities
To be responsible and accountable for assessment, diagnosis and treatment for a wide variety of neurological conditions including complex cases, in the community as an autonomous and lone practitioner to clients.
To perform advanced physiotherapeutic assessment of patients with diverse, complex and multi-system pathologies.
To work with rehabilitation therapy and care workers to deliver therapeutic programmes as part of the multi-disciplinary team.
To monitor, review and continually re-assess clients complex care plans and objectives against outcomes measures in order to facilitate achievement of their goals.
To seek guidance from more experienced Physiotherapist/ Occupational Therapy colleagues regarding the treatment and management of clients with highly complex presentations.
To work in a co-ordinated, manner, communicating effectively with other team members and the wider clinical network involved in client care.
To provide advice and education to client, carers and relatives based on clinical evidence.
To maintain up-to-date clinical written records using Rio or other designated systems in accordance with local and professional standards and also writing comprehensive discharge and progress reports to medical referrers.
To be professionally and legally accountable for all aspects of own work, including the management of clients in your care.
To identify changes in client presentation and arrange for urgent / non-urgent review by other disciplines as appropriate.
To prioritise caseload against team guidelines and standards.
To utilise communication and teaching skills to educate the diverse and multicultural population in a range of disease specific therapeutic techniques, principles and guidelines.
To maintain continuing professional development by participating in regular in service training, supervision, reflective practice and attending study days and training, giving feedback to the team where appropriate to disseminate learning.
To achieve the effective daily management of a caseload of clients, including prioritising clinical work and balancing other client related and professional duties.
Allocate work to therapy assistants as set out in team standards.
To communicate relevant medical/social information, assessment details, advice and recommendations across agencies following local and professional guidelines for sharing information and consent. This would include Consultants, GPs Social Services, other health care professionals and voluntary agencies.
To frequently exercise mental effort on a daily basis during assessment and treatment, using technical equipment as required.
To be able to operate as a lone worker in the community setting.
To be able to cope with occasional, potential stressful, upsetting or emotional situations in an empathetic manner, such as patients suffering from illnesses or pain states that may lead to anxiety and aggressive behaviour.
