We have an exciting opportunity to recruit a Band 7 Clinical Nurse Specialist in the Asthma Service. This is for a one -year fixed term contract.

The role will involve supporting the asthma service, and helping to develop and deliver pathways which result in excellent patient care and reduced waiting times.

There will be a focus on introducing an app- based tool to support patients with asthma management.

The potholder will support the wider asthma service in a variety of ways including, providing rapid access clinics, case finding, pre assessment telephone clinics, face to face clinics, community link and liaison between primary and secondary care. There will be an education component to the role, primarily for internal staff but also for our local community referrers. There will also be some audit and research opportunities. The potholder will also be responsible for mentoring junior members of the team.

The successful candidate will have at least two years' experience working with patients who have asthma. This could be in a ward, community or outpatient setting. They must be able to practice autonomously and be self-directed, but also be able to operate efficiently as part of a team. They will demonstrate an eagerness to learn and develop and have a passion for education.

Main duties of the job

Case finding within the trust, and the community via MDT's and diagnostic hubs

Support asthma management, by reviewing patients using asthma app, and using information gathered to triage referrals effectively

Pre-clinic assessment to support one-stop clinics and straight to test opportunities

New patient asthma clinic for patients who have been through MDT triage process

Deliver patient centered care by acting as a point of contact for patients and GP's ensuring there is a community link with primary care services.

Supporting patient initiated follow up (PIFU): educating patients when to self-manage and when to trigger PIFU.

Education and training. To support the education promotion of strategies for the long term and acute management associated with asthma across the community and the trust.

Undertake assessment of individuals using a range of different assessment methods, which may include physical examination, ordering and interpreting diagnostic tests or advanced health needs assessment.

Draw on a diverse range of knowledge in decision-making to determine evidence-based therapeutic interventions.

Develop pathways for high risk/high priority groups

Mentor other members of staff

The potholder will work closely with the lead clinician, Dr Roy and the wider Respiratory Nursing team. They will also work with a dedicated band 6 Asthma Nurse.

They will be supported by the Lead Respiratory Nurse Specialist.

This role will provide an excellent opportunity to develop leadership skills in relation to change management and service development, and to monitor the service and act appropriately upon findings.

Knowledge and Qualifications

Essential

• Registered Nurse Part 1, 2 or relevant section of the register
• Relevant post-registration programme

Desirable

• Teaching/Assessment programme
• Completed/completing Master's Degree or modules at MSc level
• Leadership development programme
• Management development programme
• Published work/poster presentations

Experience

Essential

• 2 years post registration experience in same specialty

Skills and Abilities

Essential

• Good knowledge of guidelines in relation to Asthma

Desirable

• Able to perform skin prick allergy testing for airborne allergies
• Able to do FeNo testing
• Able to interpret and undertake spirometry tests (ARTP accredited)
