We are looking to recruit a Band 6 Specialist Nurse to temporarily cover maternity leave within our friendly community cardiac rehabilitation team!

The right candidate will be an experienced nurse who is flexible, highly motivated and enthusiastic with excellent communication skills - cardiology experience is essential. The role will also involve the supervision of junior staff and deputising in the absence of the Band 7.

Your responsibilities would include leading cardiac clinics, patient home visits and delivering cardiac rehabilitation across the city. It is essential that you have a full, valid UK driving licence and have access to a car for work purposes.

Main duties of the job

The role would involve working with adults following Myocardial Infarction, Percutaneous Coronary Intervention, pre and post cardiac surgery (Coronary Artery Bypass Grafts, Valve Replacements and Congenital Surgery) and patients with Heart Failure. These patients often have substantial psychological, physical and emotional needs and significant co-morbidities that can complicate their recovery process.

Our primary aim is to make the transition from hospital to home as seamless as possible, preventing hospital readmissions by visiting patients and their families in their own homes to provide reassurance, support, health promotion, education and relaxation therapy.

You will be required to assess, plan, deliver and evaluate patient centred care within their own homes, community clinics and cardiac venues. You will have your own caseload and be able to prioritise and facilitate high standards of patient care. The role will also involve supporting GP's to recommend titration and optimisation of their cardiac medications.You will lead and deliver cardiac rehabilitation programs citywide, provide educational talks and health promotion, to aid improved quality of life and self-management. Your role will include supporting and supervising junior staff and work towards advanced knowledge and skills within a specialist sphere of practice.

Your role within the Team will include deputising for the Band 7 in their absence as well as working independently. You will require excellent communication and interpersonal skills. You must be flexible and adaptable and have effective organisational and time management skills. It is essential that you have good IT skills.

Qualifications & Education

Essential

  • oRegistered nurse (appropriate to branch)
  • oCurrent NMC registration
  • oDiploma plus specialist qualifications / experience within specialist area prepared to take degree level study
  • oMeets Nursing and Midwifery Council standards for student supervision and assessment (formally Mentorship) if relevant to role
  • oCurrent driving licence


Desirable

  • oDegree in health related subject


Knowledge & Experience

Essential

  • oSome post registration experience within related specialist area of practice
  • oExtended clinical practice underpinned by training supervision & assessment
  • oKnowledge and understanding of relevant NHS policy or project specific policy context
  • oHas understanding / experience of adult / child safeguarding
  • oEvidence of on-going continuous professional development
