Fylde Coast RITT is a crisis Older Adults Community Mental Health service based at the Lytham Unit, Lytham Hospital.

We are a multi- disciplinary team, with close working links with other health departments, social services and voluntary agencies.

Our service users can present with both functional and organic mental health difficulties which are often combined with complex social care and physical health needs.

RITT has 2 core functions:

o Home treatment:The provision of care coordination for patients who are experiencing acute mental health difficulties and who are presenting as high risk to themselves or others in the community and so require intensive home support from community services. Conducting Gatekeeping assessments for mental health inpatient admissions is a core component of this role as is facilitating early discharge from inpatient units.

o Care Home Liaison Team:Providing support for service users and carers for people who live in a residential home and are presenting with Behavioural and or Psychological Symptoms of dementia.

We are looking to recruit a Band 6 Community Mental Health who has an enthusiasm for and passion to work within older adult's mental health services.

The Band 6 Nurse will have the opportunity to consolidate their skills in mental health nursing as well as developing the specialist skills needed in an older adults mental health.

Main duties of the job

To actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and code of conduct.

To provide nurse leadership within the multidisciplinary team and ensure the effective assessment formulation planning, monitoring of care given to service users.

To undertake mental health assessments including Risk assessments within a multidisciplinary assessment team.To be proficient in referring to other clinicians within the service or referring to other services/agencies where necessary.

To provide a fast, responsive service to assess individual's needs.

Providing a communication structure between GP's , IAPT and non IAPT services in LCFT, non-statuary service, as a means of ensuring the highest possible standards of patient care

To provide mentoring and clinical supervision to other staff and trainees on placement.

Where the post holder is a non-medical prescriber, practice responsibly and maintain competence to effectively prescribe from the relevant prescribing formulary.

We are a 7 day a week service with operating hours from 8am to 8pm, shifts worked at present can be Early (8-4pm) Late (12-8pm), including weekend working. Flexible working can also be considered to work 4 x 9.5 hour days (8am - 6pm / 10am - 8pm daily).

Education/Qualifications

Essential

  • oRegistered Mental Health Nurse Degree oOn-going registration with the NMC. oSpecialist qualification or equivalent level in skills and development oEvidence of commitment to continuing professional development oMentorship qualification


Desirable

  • oHonour degree 2:2 or above or an equivalent professional qualification related to Health oSign-Off Mentor qualification oFirst line management/leadership qualification

Essential

  • o2 years minimum experience at Band 5 oPost graduate experience oSupervision and management of staff oAble to work as part of a multidisciplinary team oExperience of assessing, planning and implementing and reviewing care needs oExperienced in acting as a mentor/assessor of student staff oExperience of CPA Care Coordinator role
