Are you a highly motivated and experienced pharmacist with the skills to assess and manage risk, initiate and manage change and provide assurance on the safe use of medicines?
Leeds Community Healthcare NHS Trust are looking for a Professional Lead for Medicines Optimisation to join our team. You will work alongside other members of the Corporate Medicines Optimisation Team and a wider network of pharmacy professionals across the Trust. As the Professional Lead for Medicines Optimisation you will be leading and supporting a diverse range of community services to maximise the safe and cost-effective use of medicines. Your varied role will include auditing the use of antimicrobials and controlled drug handling contributing to the development of Patient Group Directions and shared care frameworks for specialist medicines advising on the sustainability aspects of medicines use delivery training and supporting our digital prescribing programme.
You will undertake the duties of Medication Safety Officer (MSO) for the Trust, leading the medication safety programme for the organisation, working with MSOs across Leeds and West Yorkshire to share best practice. You will also support the Accountable Officer for Controlled Drugs to discharge their duties and deputise for the Head of Medicines Optimisation as required.
Main duties of the job
The Medicines Optimisation Team provides and develops the strategic direction for how medicines are used in Leeds Community Healthcare Services in accordance with national and local priorities and to ensure that there are safe and effective systems in place within services for the management of medicines.
Our mission is to ensure the safe and effective use of medicines across the Trust within a legal framework by providing efficient and reliable solutions to clinical services. Around two-thirds of our clinical teams use medicines in some way, and need advice, expertise and guidance on prescribing, administration and supply of medicines. Our work takes us to all areas of the Trust where medicines are used including community bedded units, patients homes, clinics and secure estates.
