The post holder is a key member of the in-patient multi-disciplinary team, providing assessment and treatment to older people with mental health problems working within a recovery focused service. There is a need to demonstrate a commitment to teamwork.

Personal and professional development is a requirement of the post and will be supported through both internal and external training opportunities.

The post holder will be required to work rotational shifts and be required to work a proportion of shifts which include evenings and weekends and may be required to work in other areas of the Trust to meet service needs.

You'll be joining a supportive and friendly team that will make your transition into the ward an enjoyable experience.

You will be supported in your own professional development with opportunities to undertake a range of CPD and funded University BSc/MSc courses. You will have the opportunity to participate in research and Quality Improvement projects.

Main duties of the job

Pyrland operates as 2 separate wards both wards have 14 beds providing assessment and treatment for people over the age of 65 across Somerset.

Pyrland Ward 1 specialises in the treatment of older adults with a range of functional mental health needs. Pyrland ward 2 provides specialist assessment and care for people with organic mental illness (i.e dementia).

Part of the role includes leading a team during the shift alongside a fellow RMN, providing evidenced-based care to our clients and working collaboratively with our community team colleagues to ensure that the admission and discharge process is effective.

To create and maintain professional supportive relationships with all members of staff, with other professionals and agencies to enhance recovery.

Planning and Organisation

*To lead the clinical management of designated service users in assessing, implementing and planning nursing care.

*To ensure that junior staff and students are actively engaged in patient activity.

*To observe, report and record changes in the patient's physical, mental health and behaviour.

*To work with the MDT in planning and delivering care.

*To maintain working knowledge of legislation and guidance pertaining to mental health and learning disabilities.

*To be responsible for the administration of prescribed medication in line with Trust policy and drug administration standards.

*To work within and manage the legal and professional frameworks relating to consent, capacity, deprivation of liberty and the Mental Health Act.

*To understand the range of interventions available, the evidence-base and potential outcomes for service users.

*To ensure that all clinical and risk assessments are completed and reviewed in line with Trust policies on data collection.

Responsibility for Patient / Client Care, Treatment & Therapy

*To establish, maintain and end purposeful therapeutic relationships with patients.

*To identify and provide and supervise therapeutic activities, emotional support and stability to patients to ensure effective engagement whilst maintaining professional boundaries.

*To undertake required physical observations including blood pressure, temperature, respirations, blood sugars, urinalysis, weight and height and report exceptions appropriately.

*To identify, provide and supervise the necessary physical care to patients to help them complete daily living activities. Personal care and daily living activities include personal hygiene, elimination, diet and fluid intake. clothing and laundering, manual handling, personal environment and maintaining rest and activity.

Qualifications

Essential

  • *Relevant professional qualification and registration e.g. RMN
  • *Appropriately registered with the NMC as per post requirements


Desirable

  • *Relevant teaching assessment qualification


Experience

Essential

  • *Knowledge of NMC code of professional conduct
  • *Evidence of continuing professional development and to have undertaken post registration training
  • *Ability to demonstrate and evidence post-qualifying experience
  • *Experience of carrying out supervision to junior staff
  • *Experience of assessing and managing clinical risk.


Desirable

  • *Experience of electronic patient records/RIO
