Registered Nurse, Stroke Recovery Centre, Williton Community Hospital
This is an exciting opportunity for an innovative nurse or Nursing Associate with a passion for Stroke care to join our integrated team on Meadow Ward Stroke Recovery Centre, Williton Hospital.
We offer specialist Stroke rehabilitation beds with the goal of re-enabling our patients to live their best life at home with the support they need to maintain this. For those that can't go home, we facilitate discharge to an appropriate care setting & continue our focus to maintain as much independence as possible. In addition, Meadow Ward offers End of Life care in our beautiful Grace's Room.
You must be a Registered Nurse who communicates well at all levels, have high clinical standards & the ability to work as part of a multi-disciplinary team providing holistic care, ensuring all patients are treated with dignity, respect & compassion.
Flexible working on internal shift rotation, working both days & nights over a seven-day period, including bank holidays. We offer training opportunities & preceptorships for newly qualified nurses.
Main duties of the job
The role involves supporting the Sisters in the daily running of the ward, acting as a shift coordinator as required, supervising colleagues & promoting a safe environment within the unit. You will be responsible for all aspects of patient care, from admission through to discharge, using a computerised record keeping system.
Successful applicants will attend family meetings, undertake ward rounds and be involved in multi-disciplinary team meetings. These all contribute to the decision-making process, goal-setting and care planning that facilitates a safe discharge from hospital. You will provide Stroke/End of Life nursing care to our patients, ensuring they are all treated with dignity, respect and compassion.

The development, implementation and evaluation of programmes of care, including discharge planning for each patient. This will include the specialist assessment of care and health education needs of stroke patients.
To be designated to take charge of the unit in the absence of the ward manager.
To work in accordance with the Unit's philosophy of care and contribute towards achieving its objectives.
To maintain effective communication and promote professional caring and partnership.
To participate in clinical supervision as appropriate and to teach and act as a facilitator/mentor/preceptor and role model to less experienced staff, developing expertise in stroke nursing.
