This is a fixed term community nursing and care coordinator role within Tower Hamlets Community 24/7 service pilot. The pilot ensures delivery of high quality, modern, patient-centered services. This is a pilot project aimed to establish and test out a new model of care that breaks down current barriers to care, is nonrestrictive, co-produced, culturally sensitive and accessible. The service will run 24hrs a day, 7 days a week for people residing in the catchment area offering open access to a range of psycho-social- medical Interventions at every stage of their wellness journey, this includes short term Hospitality stays for those in crisis, and community based offers that are embedded in the neighbourhood.

The Team is multi-disciplinary and multi-agency and committed to co-production, facilitation and liaison with service users, local residents, local VCSE, health and social care services in order that people with co-morbid physical and mental health conditions, experience good outcomes for their mental health. Additionally the team provides specialist input where this is required.

Main duties of the job

The role involves, as part of a Multi-Disciplinary team

• setting up, implementing and review appropriate care plans and to support and advise relatives and care givers.

Make sure that co-production is at the core of the role

Ensuring that compassion and empathy are central to care delivery

• promoting the needs of service users with mental health needs and to offer and provide training and education whenever possible.
• in conjunction with the crisis pathway services, provide a safe and effective urgent community assessment and treatment response service.
• undertake comprehensive assessments, including risk formulation, effectively and efficiently
• formulate and agree crisis safety care plans and to support and advise relatives and care givers.

We are one of six pilot sites in England and intend to reshape care.

Qualifications

Essential

• RMN/RGN Educated to degree level
• Mentorship Training or equivalent
• Recognised post graduate qualifications in care of the adult with mental health problems
• Evidence of regular updating and ongoing development of role and skills

Knowledge

Essential

• Thorough understanding and working knowledge of mental disorder and the treatment options. . Ability to assess manage complex and high risk situations effectively and to the benefit of service users and carers
• Good report writing skills and ability to compile complex case recording ad report presentation, including legal documents i.e. social circumstances reports, court reports etc.
• Ability to manage and co-ordinate professionals, teams and services and mobilise compile care plans in partnership with service users, carers and others provision where appropriate. . Assessment skills and the ability to
• Assertiveness and engagement with other professionals, able to make reasoned arguments and represent own professional views and perspectives.

Experience

Essential

• oSignificant post-qualification experience, which must be within mental health services .
• o Experience of working within a multi-disciplinary environment/community experience. .

Desirable

• o Experience of working within an NHS/Local Authority environment and an understanding of the issues/ policies/ practice within these environments.
