We are pleased to have an exciting opportunity in a newly created role for 12 months within the CPFT Operations Hubs.

The role will be centred around the early identification, recognition and assessment for patient flow within the three Intermediate Care services. With a focus on internal constraints to patient flow, working alongside services to embed robust processes to support patient flow.

Main duties of the job

The post holder will be a member of the Operations Hub working effectively with the three Intermediate Care Services (Intermediate Care at home, Intermediate Care in a Care Home & Intermediate Care in an In-patient rehab ward) to deliver quality patient flow services. There is a specific focus on the internal processes, including red to green, long length of stay and internal constraints to optimising patient flow.

The post holder will deliver and establish effective multi-disciplinary working within the Intermediate Care Services, this will include:
  • overseeing the smooth running of the discharge process
  • delays in all elements of the discharge process
  • long length of stay
  • clinical audit
  • standards and outcomes for patients
  • other quality aspects of patient flow.
  • To proactively work alongside the Intermediate Care Services to focus on the internal patient flow journey
  • Provide advice and support to the wider members of the Intermediate Care Services and Multi-Disciplinary teams

Job responsibilities

  • Take a lead role in re invigorating the red to green daily process across the Intermediate Care Services, including data collection
  • Sharing best practice across the Intermediate Care Services for internal patient flow process
  • Responsible for ensuring that no patients are on the Stranded Patient Tracker list (PTL) due to internal reasoning
  • Escalating concerns and constraints
  • Clear, succinct and precise documentation in patient records


Person Specification

Education & Qualifications

Essential

  • NMC or HCPC Registration
  • Degree or equivalent experience


Desirable

  • Post qualification continuing professional development


Experience

Essential

  • Minimum 2 years' experience as a senior professional.
  • Evidence of successful working within multi-disciplinary teams
  • Evidence of managing workload and ability to prioritise


Desirable

  • Experience of working with wider stakeholder groups
  • Experience of staff management


Skills & Abilities

Essential

  • Evidence of leadership ability, able to influence, involve and develop individuals and teams
  • Decision making and problem solving skills
  • IT literate
