We are seeking an experienced manager to support the operational management of our Home First teams in Greenwich.
Home First aims to address the increasing demands on local health and care services due to an ageing population and multiple co-morbidities. Many people, especially older people and those with a long-term illness, require regular health and care in both acute and community settings. The vision for the Home First approach is for high-quality, flexible, safe, holistic and rapid interventions which provide a real alternative to hospital care.
Main duties of the job
Reporting to the Adult Community Physical Health Service Manager for Home First (Greenwich), the post holder will be responsible for the management, leadership and supervision of the teams constituting the Home First services. This is an integrated role working across both health and social care and the teams have multiple pathways to prevent hospital admission and promote early hospital discharge, with the Joint Emergency Team and Community Rehabilitation and short term assessment teams being central to this approach. The services utilise remote monitoring technology and we are currently building an assistive technology enabled care service with The Royal Borough of Greenwich.

We are the largest NHS provider of prison health services providing healthcare to prisons within Devon, Dorset, Bristol, Wiltshire and Gloucestershire, Kent and South London. Experience
Essential
- Proven experience of and demonstrable achievement in managing health/social care or other people related service
- Management of resourced (both people and financial) with a good track record of change management, efficiency savings and financial recovery plans
- Experience of producing high quality reports
Desirable
- Experience of managing a diverse team (multi-site)
- Measurable track record in involving and motivating staff to improve performance
Skills and Knowledge
Essential
- Ability to present highly complex and emotive information clearly and openly with staff
- Ability to analyse and interpret performance and financial information
- Awareness and understanding of the Health & Social Care Act

- Ability to see through tasks to their successful conclusion within timescales and budgets
