The Urgent Community Response (UCR) practitioners provide interventions within 2 hours and over a 48 hour period to prevent hospital admissions. The UCR practitioners will link in with the wider community teams, including ARCH (Acute Response and Rehabilitation in the community and hospital service), CCT (Community Care Team), specialist community teams and voluntary sector organisations. Services are provided and managed by Harrogate and Rural Alliance (HARA), a partnership between Harrogate and District Foundation Trustand North Yorkshire Council.

The post holder will begin as a band 6, and then be supported by the trust to develop the required competencies to progress to Band 7. The post holder will be expected to complete the requirements listed within a reasonable timeframe. Alternatively if you are an existing Band 7 practitioner with the required experience, we would very much like to hear from you, as this could be the role for you.

Main duties of the job

  • Visit and assess patients who have been referred to the Urgent Community Response (UCR) practitioner, create a treatment plan and work within the Community teams to prevent hospital admission and handover care within 48 hours
  • Practitioners will cover 0800 - 2000, 7 days a week, with the ultimate aim of becoming 24 hours. The post holder will work across these hours, alongside 3 WTE substantive practitioners already in post.
  • To work towards completion of a competency document supporting clinical skills (please see attached document) in order to support high acuity patients in their usual place of residence.

    Qualifications, skills, experience

    Essential

    • Required clinical skills
    • Work independently and clear decision maker
    • Evidence of positive risk taking
    • Evidence of professional development
    • Either Registered Nurse or HCPC Registered Paramedic/AHP
    • Extensive knowledge of Health and Social Care Drivers
    • Able to drive and have own vehicle in order to meet service needs


    Desirable

    • Experience in an urgent care facility e.g. ED/Minor Injuries
    • Experience in discharge
    • Independent Prescriber
    • Training at advanced clinical management level
