We have an exciting opportunity for the right person to join our team and play a major role within the leadership team, leading the clinical element of the Reablement service.
The Reablement service provides support and therapy input to patients at the point of discharge from hospital, with the journey spanning the recovery, rehabilitation and reablement phases. We also provide Reablement support to community patients, to prevent or reduce the need for permanent or ongoing packages of care through focused therapy and Reablement input. Our team presents a unique opportunity to undertake very close working with our county council social care colleagues, with a truly multidisciplinary team comprising of occupational therapists, physiotherapists, nurses, band 3 and 4 therapy assistants (some of whom are currently undertaking OT apprenticeships), social care workers, assessors, and Reablement assistants.
Main duties of the job
Accountable to the Deputy Head of Homefirst, you will support strategic leadership operationally on a day-to-day basis. The role entails the operational management of the clinical element of the service, which includes the line management of six Band 7 direct reports and Band 3 administrator recruitment oversight Quality and performance monitoring and service improvements.
The post holder will need to take part in system calls regarding patient flow at times and support with the escalation of delays where needed. You will work closely with the Deputy Head of Home first, fellow Integrated Community Service Leads as part of our Senior Management Team, Reablement leadership and operational teams.
The post holder will be involved in an extremely exciting and challenging period of service development, developing closer working and operational alignment with partner organisations.

- Significant post graduate leadership experience in core areas
- Proven record of implementing change and developing services
- Experience of managing a budget

- Evidence of management experience at band 8a team leader/ team manager level or equivalent
- Experience in the management of long term conditions and admission prevention schemes.

- Experience of working across health and social care
Qualifications
Essential
- A current professional qualification (RMN, RGN, OT, Physiotherapist)
- Registered to practice in the UK.
- First line management qualification or equivalent experience
- First degree or equivalent in health care subject
- Evidence of on-going CPD
- I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
