We have a 12 month fixed term role within our Department. Consideration will be given to a secondment, this must be discussed with your line manager prior to applying.

Liverpool University Hospital Foundation Trust are working in collaboration with Cheshire and Merseyside Cancer Alliance (CMCA) to ensure that all patients with a cancer diagnosis have access to personalised care interventions.The aim of this project is to ensure that Personalised Care and Support Plans (PCSP) (based on Holistic Needs Assessments (HNAs)) are available for all cancer patients, and data is submitted to Cancer Outcomes and Services Data set (COSD).

We are looking for an innovative, self-motivated individual with excellent leadership, team working and organisational skills. A nurse with clinical experience in the delivery personalised care would be an advantage.

These posts are a great opportunity to build on your existing project management skills, knowledge of data collection and use of information systems for example the Somerset Cancer Registry. Experience in the delivery of projects within the NHS would be an advantage.

The successful post holder will lead on a digital personalised care project. They will lead on developing Standard Operating Procedures (SOP) / Policies for HNAs and PCSPs and be involved in the development and delivery of training and education for personalised care interventions.

Main duties of the job

The successful post holder will lead on a digital personalised care project.

The highly skilled individual will develop an improvement plan with clear key performance indicators to demonstrate quality of personalised care interventions, monitor activity and improve data compliance across all four using a wide range of clinical systems such as the Somerset Cancer Registry (SCR) and My Care plan

The highly skilled individual will develop a plan to improve the quality of data compliance for personalised care interventions across Trust and will work with the BI team to develop an interactive dashboard on all Personalised care interventions using a wide range of clinical systems such as the Somerset Cancer Registry (SCR) and My Care plan

The post holder will be expected to work closely with Lead Cancer Nurse's and the Cancer Workforce and Business Intelligence teams across the four organisations to support the implementation of local improvement plans for national data collection via the Cancer Outcomes Data Set

Ensure the roll out of digital platforms for the delivery of holistic needs assessment and personalised care plans and end of treatment summaries

Work with BI Teams to optimise digital interfacing with relevant clinical systems and develop an interactive dashboard for Personalised Care Interventions in collaboration with Cheshire and Merseyside Cancer Alliance

The successful candidates must be willing to undertake frequent travel between various locations to carry out the duties of the post as required.

Person Specification

Qualifications

Essential

  • Masters Degree or equivalent
  • Formal project management qualification (PRINCE2 Practitioner or MSP Foundation) or ability to demonstrate substantial experience and success in delivering projects


Desirable

  • ITIL Foundation qualification


Experience

Essential

  • Specialist experience of managing a series of complex, large inter-related projects or substantial clinical experience for those projects requiring clinical expertise
  • Experience of managing a project team
  • Substantial experience of using specialist Microsoft Office applications
  • Working with colleagues at all levels including clinicians
  • Working on unfamiliar topics requiring rapid assimilation of new technical knowledge
  • Teaching and presentation skills
  • Experience off affecting change in an organisation with no direct line management responsibility
  • Analyses of non routine data, interpretation and resolution

  • Experience of working in a NHS environment
