An exciting opportunity has arisen to become an Advanced Clinical Specialist Physiotherapist within East Cheshire NHS Trust's community team.

The role will involve working within Chaw Care Community based at Handforth Health Centre. We welcome applications from Physiotherapists with a keen interest in working within an integrated community team, with proven clinical expert experience in bed based rehabilitation and general community rehabilitation. The successful applicant would be the clinical lead for the team, helping to direct and develop the current team members.

Hours per week: 30

Main duties of the job

  • To provide clinical expertise, making decisions about the best course of action for patients' care (both in bed-based rehabilitation and within the community setting).This will involve assessing patients and providing personalised rehabilitation plans, whilst working alongside multiple agencies, to determine the best management plan for patients.
  • To provide leadership and clinical supervision to junior staff members.
  • To challenge multi-disciplinary team discharge decisions to ensure all patients in cluster beds meet the Criteria to Reside
  • To deliver supportive patient rehabilitation programmes, in ways that facilitate behavioural change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, and that ensure patients can be maintained safely within their own homes, if possible.
  • To maintain and develop links with the wider multi-disciplinary team to support and improve community rehabilitation pathways across East Cheshire.

Qualifications

Essential

  • HCPC (Health and Care Professions Council) registration
  • Relevant professional degree /diploma / MSc pre-registration
  • Clinical Educator training

  • Proven experience of working in the community setting

  • Membership of a relevant professional body & related specialist sub-groups
