Are you a highly, experienced clinical pharmacist looking for a new and unique way to develop your clinical specialism, whilst playing a pivotal role in delivery of transformational change across 3 acute trusts in a provider collaborative?

If so, we have an exciting opportunity for an enthusiast and motivated pharmacist, with an interest in Gastroenterology, to join our Pharmacy Team at Great Western Hospital on a 12m, fixed-term basis as an 8a Advanced Pharmacist (Gastroenterology).

You will initially focus on working across BSW Acute Healthcare Alliance to deliver some key, specialty- specific, biosimilar switch savings, but will also work towards establishing the role of a Gastroenterology Specialist Pharmacist within the Gastroenterology team at Great Western Hospital, including non-medical prescribing in out-patient / day hospital settings and supporting medicines optimisation and medicines effectiveness in both in-patient and outpatient services.

By demonstrating the value of this post to both our Trust and the wider BSW Acute Healthcare Alliance at the end of its fixed-term, you will play a key role in influencing our future service developments and, potentially, the establishment of a similar permanent post.

Main duties of the job

1. Act as an advanced Pharmacy practitioner for the gastroenterology, to plan the care and identify areas of concern related to medicine use within this specialty.

2. To be an Independent Prescriber (IP) and to continue to develop the role of pharmacist independent prescriber within the Trust.

3. To provide a specialist clinical pharmacy service to patients in gastroenterology wards and out-patient areas in line with clinical pharmacy standards and objectives.

4. Clinically checking all prescriptions before dispensing and referring to the prescriber any unusual doses, potential drug interactions or incompatibilities for confirmation or revision before authorising dispensing.

5. Assist in clinical decision making to ensure patient safety and medication effectiveness through the medicines optimisation agenda.

6. Discussing with patients any issues about their medication, and liaising with other health care professionals in Primary and Secondary Care as appropriate.

7. Reviews results of external audit related to patient care and develop action plan as necessary.

8. Fulfil the role of Responsible Pharmacist in the dispensary when register able activities are taking place, when required as part of the rota or out of hours, as defined in the relevant Trust guidance document. To be aware of the requirements of this role and the relevant standard operating procedures, and to complete the record forms as require.

Specialist Lead

1. Work in collaboration with system wide colleagues to ensure best medicines value for gastroenterology

2. To lead and develop the clinical pharmacy service to gastroenterology specialty within the Trust. This includes setting and monitoring quality standards for clinical pharmacy services.

3 To introduce, maintain and develop a team and specialty-based model for clinical pharmacy service provision.

4. To act as the point of liaison for the specialty and lead on pharmacy-related aspects of the development of services, medicines introduction, horizon scanning and specialist information provision.

5. To regularly meet with the Pharmacy Clinical Services Manager and Medicines Effectiveness team to develop a strategic plan for clinical pharmacy services.

6. To provide the Pharmacy Clinical Services Manager and Medicines effectiveness lead with details of output, performance and activity of the clinical pharmacy service within neurology speciality at specified time intervals in a format agreed by the Pharmacy leadership team.

7. To actively participate in trust wide medicines assurance, governance and specialty meetings to promote the safe effective use of medicines and role of Pharmacy in medicines optimisation.

8. To prepare business cases to support clinical Pharmacy service developments within the speciality.

9. To collaborate in the development of Integrated Care Pathways, clinical guidelines, Standard Operating Procedures, Patient Group Directions and any other required policies within the speciality and Pharmacy department.

10. To support the investigation of medication related safety incidents and formulation/delivery of actions plans across the Trust to address problems identified where appropriate.

Person Specification

Qualifications

Essential

  • M Pharm Degree or equivalent / General Pharmaceutical Council (GPhC) registration
  • Postgraduate diploma (Diploma or MSc) in clinical pharmacy, community pharmacy or similar
  • Keeping professionally up to date including CPD
  • Non-Medical Prescribing Qualification


Desirable

  • Management experience
  • Experience working in out-patient clinic
  • Member of Royal Pharmaceutical Society (RPS)


Experience

Essential

  • Highly experienced as qualified pharmacist
  • Experience working across departmental and organizational boundaries
  • Knowledge of financial systems and structures
  • Experience in Gastroenterology


Desirable

  • Previous experience as specialist pharmacist
  • Experience of service development


Skills

Essential

  • Good communication, verbal and written eg SOPs
  • Presentation skills - teaching and repost writing
  • Use of financial systems to generate reports
  • Good clinical knowledge backed up be extensive ward based experience
  • Able to priorities workload and delegate and manage time effectively


Desirable

  • Use of databases to collate information
