Are you a dynamic and compassionate Occupational Therapist looking to make a significant impact on patients' lives during critical moments? We are seeking a skilled and dedicated Rapid Response Occupational Therapist to join our healthcare team and provide expert care to patients requiring urgent Occupational Therapy interventions.

Main duties of the job

You will
  • Have an opportunity to shape / develop therapy roles within the Rapid Response Team.
  • Work collaboratively with health and social care professionalsacross the borough and Integrated Care System.
  • Lead Clinical decision making and undertakeenhancedholistic assessment of patient/family needs in partnership with the patient and their family/carer.
  • Triagereferralsfromotheragencies ordisciplines
  • Co-ordinate intensive and complex packages of care with other multi-disciplinary agencies.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).

Job responsibilities

  • Conduct rapid assessments of patients' functional abilities and barriers to independence.
  • Create customized intervention plans to address immediate needs and promote functional recovery.
  • Deliver prompt occupational therapy interventions to enhance patients' quality of life.
  • Collaborate closely with interdisciplinary teams to ensure holistic patient care.
  • Maintain accurate and timely documentation of assessments, interventions, and progress.
  • Educate patients and their families on adaptive strategies and techniques.
  • Stay updated on emerging practices to ensure the highest level of care.
  • work in collaboration with Virtual ward to ensure patients are receiving appropriate therapeutic care.
  • respond urgently to patients requiring unplanned care as per Urgent Care Response guidance.

Qualificatuions

Essential

  • Bsc or equivalent in Occupational Therapy
  • HCPC Registration


Desirable

  • Member of RCOT
  • Post graduate training in relevant field

  • Experience of working in admission avoidance services , or A and E


Skills and knowledge

  • Demonstrates sound IT skills eg Microsoft packages excel, word, teams
  • Demonstrates good communication and written skills
  • Ability to travel across the borough or valid uk driver 's licence
