Main duties of an Urgent Care Community Nurse
You'll will be part of a supportive friendly team and be responsible for a varied caseload, preventing emergency hospital admissions and treating patients in their homes. Referrals are received from 111, hospice, South Western Ambulance Service NHS Foundation Trust (SWAST), and the Community Nursing team. The team operate shifts between the hours of 4.00pm-8.00am.
In this role youll be able to hone your skills across a wide range of conditions management, end of life care and new illness and injury treatment. Working on your own and with Healthcare Support Workers, and the wider Community Nursing team, you will gain experience teaching and working autonomously.
This role will require you to have a driving licence, however you will not be expected to use your own car for visits, as youll be able to collect a pool car at the beginning of each shift.

You will have excellent communication and assessment skills and will be responsible for assessment, planning, implementation and evaluation of personalised patient centred care in partnership with our patients and those important to them.
Working Pattern:
Working patterns part-time - 22.5 hours per week
Depending on the role there may be a requirement for weekend working on a rota system.

Depending on your shift pattern and working hours, you will receive additional enhancements to your pay as follows:
time plus 30% for weekdays from 8pm to 6am
time plus 30% for Saturdays (midnight-midnight)
time plus 60% for Sundays and Bank Holidays (midnight-midnight)
