The LiveWell team are looking to recruit a team leader to work on a new and exciting project to support residents of Cardiff and the Vale at risk of Falls. The main duties and responsibilities will be around managing the Community Falls programme and supporting the MDT to deliver high value high impact interventions. The post holder will work collaboratively with CRT's and community partners across the health boards geographical area. They will be managing a team of support staff who they will support to deliver the new Falls programme

- Recognised qualification degree / equivalent in physiotherapy/ Occupational Therapy or Nursing
- HCPC or RN Registration
- Evidence of Masters level learning or relevant higher-level qualification and experience
