An exciting opportunity has arisen within the Home First Unplanned Care Nursing Team based in Milton Keynes. Home 1st provides a 24/7 seamless service in supporting unplanned care and admission avoidance in the community. We are currently recruiting band 5 Registered Nurses to work within Home 1st Unplanned Care.
We are looking for clinicians ideally with community experience to join our nurse lead team. When you join our team you will contribute to our goal of developing our service beyond the traditional scope of Nursing in response to the challenging health care needs that we are facing nationally.

Main duties of the job
To be professionally and legally accountable for all work To work within the standards and guidelines of the team, the Trust, and The NMC Code.
To deliver nursing care to patients within their home, that is technically excellent, expressly personal and ensures patients and carers dignity is enhanced.
To undertake skilled nursing activities directly related to the assessment, planning and evaluation of patient
To response in a timely manner to unplanned nursing care referrals.
To recognise the deteriorating patient and escalate appropriately.
Management of patient care ensuring that risks are adequately assessed, minimised, translated into appropriate care plans and reported across the organisation as
Manage the effective coordination and continuity of patient care working closely with external agencies and community
To ensure that clinical practice is contemporary to meeting the needs of patients and to ensure each patient has a comprehensive assessment of need, including risk assessment and their care is culturally sensitive and addresses issues of diversity.
To provide health information and health promotion for patients and their
To work closely with the acute trust, GP, nursing / care home, community nursing services and therapy colleagues to prevent inappropriate hospital
To evaluate your practice through audit and outcome measures with support of senior colleagues when
To maintain and develop specific departmental duties as designated by clinical
