We are looking for two experienced nurses to join our Respiratory team, to provide excellent inpatient and outpatient care cross site (1 role is permanent and the second is to cover maternity leave for a 12 month fixed term contract). The post holders will provide clinical expertise both in inpatient and outpatient care, and will join a dynamic team based at WMUH and Chelsea & Westminster Hospital.
Main duties of the job
The respiratory clinical nurse specialist will be the main specialist nurse expert providing support to the wards with respiratory patients. This includes patients with Asthma and COPD, patients requiring non-invasive ventilation, and those requiring assessment for continuation of services in the community including long term oxygen therapy and/or pulmonary rehabilitation.
The post holder will be a key member of the multi-disciplinary team and will participate and contribute to multi-disciplinary meetings. They will act as a role model and leader to all staff dealing with respiratory patients, initiating treatment, and taking emergency action where appropriate.
The CNS will provide advanced clinical skills and advice in all areas of specialist nursing care to colleagues, patients and their families. They will demonstrate a sound understanding of the issues related to the identification, assessment, diagnosis and treatment of respiratory patients, with the formulation and alteration of treatment plans, monitoring of care and the discharge and follow up of patients. They are expected to facilitate early supported discharge scheme and will case find patients suitable for Virtual Wards. They are also expected to refer as appropriate and should be actively involve in local and national audit/s.

Working as an integral member of the Respiratory team across Chelsea & Westminster and West Middlesex University Hospital
Identifies at an early stage, following liaison with appropriate clinical staff, patients who require support from the respiratory CNS
To apply professional nursing knowledge in order to undertake high quality assessments assisting in the diagnosis and management of patients who meet the early supported discharge for COPD criteria
To assess and interpret clinical conditions and appropriate outcomes using a patient centered approach that promotes independence and well-being
To refer to other disciplines/professions when required
To accept referrals from other disciplines e.g. medical / surgical teams
To support the patient, family and MDT in understanding any continuing care plans and treatment
To ensure that documentation is maintained to a high standard and to be a competent user of and participate in the ongoing development of the Electronic Patient Record
To support the ward/ department team to facilitate seamless transitions of care/ discharge by liaising with primary care health and social teams as required
Case-manage patients with Asthma and COPD at the Hospital. This will include respiratory and non-respiratory wards, acute and outpatient areas. Duties will include history taking, physical examination, ordering, performing and interpreting relevant investigations and coordinating discharges
As part of acute work this may involve supporting the initiation and optimisation of acute non-invasive ventilation
Admission avoidance from the Emergency Department by assessing and providing care plans for discharge.
They will work with the existing general multidisciplinary team, but be able to work independently to complement the clinical teams in this area. Work may include attending ward rounds and multidisciplinary team meetings if deemed appropriate.
To contribute to the achievement of the COPD best practice tariff
Communicate with patients and their next of kin
Promote patient and public involvement in the development of respiratory services
Undertake clinical audit and research
Provide education to students, nursing and medical staff and develop the curriculum as required
To make independent clinical decisions based upon patients needs
Undertake comprehensive assessments of patients including physical, mental and pharmacological considerations
Develop relationships with colleagues across the hospital and the inter-professional agencies
Provide education and mandatory training and contribute to the development of protocols as necessary
Keep up to date with professional development and mandatory training
Keep accurate notes and records
Maintain high standards of care
Acquire and develop new skills relevant to specialist role
Responsibility for Patient/Client Care
To act in accordance with the NMC Code of Professional Conduct for Nurses, Midwives and Health Visitors and to be accountable for own clinical practice and professional actions at all times. Ensure continued and effective registration with the NMC
Organise the clinical management of individual patients, utilising clinical knowledge and advanced skills to assess, diagnoses, plan, implement and initiate changes where required. This will include monitoring and evaluating the response to treatment through analysis and interpretation of clinical tests and investigations and the discharge of the patient
Respond to emergency situations, utilising clinical judgement and decision making to initiate appropriate interventions and treatment for patients
Initiate drug therapy/medication within the parameters of agreed clinical guidelines and in accordance with present legislation for the supply and prescribing of medication
Monitor and evaluate clinical outcomes of patient centred care involving the patient and their carers wherever possible
Participate and contribute to the decision-making process in patient management
Take every opportunity to promote the health of patients and initiate health education strategies as necessary
