Children's Community Nurse EPUT Caring for Children in the Community Band: 6 Hours: 33.75 hours per week

As our name suggests, we are actively putting care at the heart of the community and you have a vital role to play as a key member of our Children's Community Nursing Team.

Main duties of the job

We are looking for a highly motivated, innovative and flexible Children's Nurse to join our expanding community team. The successful candidate will be an RSCN or RN Child with acute paediatric experience they will have an in-depth knowledge of providing family centered holistic nursing care .The individual will be organised and be an excellent communicator able to cross boundaries with the MDT and families alike.

The role includes collaborative working with Health Visitors, School Nurses, GPs and the Paediatric Liaison Health Visitor to ensure the delivery of relevant, co-ordinated and high quality care solutions for children and families within the home setting.

The service operates a shift system provide care during the period of 08:30 to 18 30pm seven days a week.

The Children's Community Nurse (CCN) is a member of the Children's Community Nursing Team providing specialised and skilled nursing care of children in the community as designated by the Modern Matron/Team Leader of the CCN service.To reduce unnecessary hospital admission and promote hospital avoidance and to provide specialist nursing support in the childs own environment in partnership with the child, young person , family, carers and other professionals.
