An opportunity has arisen for a highly skilled, enthusiastic and motivated individual to join our Telford Locality team.
The service consists of multi-disciplinary teams that include Physiotherapists, Occupational Therapists, Assistant Practitioners and Rehabilitation Support Workers and working closely with the nurses and paramedics in the Rapid Response team. The aim of the MDT is to promote independence through rehabilitation and crisis management with patients in their own home or within a short term care setting to prevent unnecessary hospital admissions and support timely hospital discharges.
The role includes delivery of efficient and effective services in line with local, national and professional agendas.

Main duties of the job
You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal focused rehabilitation and reablement programs, as well as complex discharge planning experience. An understanding of Health and Social care provision and ability to work closely with other organisations and stakeholders is essential.
You will need to demonstrate high standards of evidence based clinical practice. You must have excellent interpersonal and communication skills. You should be able to prioritise your workload, manage your time effectively, and work flexibly within the team.
We are looking for enthusiastic, motivated, and conscientious & experienced Physios to work with the established therapy teams. We are a dynamic, developing organisation with well recognised rehabilitation services providing education, advice, support and rehabilitation for patients.
