An exciting and unique opportunity has arisen for an enthusiastic, flexible and highly motivated band 6 Physiotherapist to join the Eden ICC team. Formally known as the Community Rehabilitation Service we now provide the opportunity to work within a fast paced and dynamic service both developing a rapid response element to the team alongside providing rehabilitation across Eden ICC.
Where else can you enjoy one of the most beautiful areas of the UK around the edge of the Lake District whilst you work?!
Eden ICC provides admission prevention, supported discharge, specialist assessment and rehabilitation for patients with a wide range of long and short-term conditions. We work closely with our health and social care colleagues for care and advice.
The successful candidate will need to be motivated and able to demonstrate excellent assessment and communication skills as well as good team working.
Main duties of the job
Main duties
Eden ICC demonstrates fantastic multidisciplinary team working by supporting D2A's (Discharge to Assess) and providing rehabilitation alongside rapid response care. You will work particularly with frailty patients, those who are at risk of or who have fallen and patients with long term conditions.
You will need to have excellent communication skills, be able to demonstrate the ability to work in challenging environments and be committed to working autonomously, providing evidence based assessment, treatment and rehabilitation.
This post offers the chance to develop clinical, supervisory and managerial skills. We support the education of physiotherapy students from local Universities, and you should be able to demonstrate commitment to your own and other's professional development.
We work closely with Commissioners, GPs, Acute Services, Community hospitals, CHOC, Cumbria County Council Adult Social Care and third sector organisations. A key focus is to provide care in patient's own homes to prevent hospital admissions, facilitate early discharge and where possible to enable patients to manage their own conditions and remain in their own homes, enhancing both their independence and quality of life.
Patients can be seen as in-patients (in Community Hospital beds) but the majority of work is based in patient's own homes (including residential and nursing homes).
