We are seeking a motivated, dynamic and passionate Band 5 Community Nurse to work within our Mid Bedfordshire Primary Home Care Team based in Ampthill Health centre. This is an excellent opportunity to develop your knowledge and skills within the community nursing setting. You will have the opportunity to be involved in facilitating self-care for patients with long-term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing. You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams.

Main duties of the job

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. Newly qualified nurses who are passionate about a career in the community are welcome to apply and will be supported through preceptorship.

Job description

Job responsibilities

You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-21:00.

Essential

  • oCurrent UK driving license and access to a car
  • oAbility to travel across sites and across Trust boundaries to meet patient needs and attend meetings, etc.


Education/Qualification/Training

Essential

  • oRegistered General Nurse
  • oDemonstrate evidence-based practice and up-to-date clinical skills.


Desirable

  • oEvidence of post-basic professional development
  • oPost-registration qualification in a relevant clinical field.
  • oRelevant teaching/mentorship qualification or commitment to undertake
  • oPost-registration nursing experience, including community experience


Physical/mental and emotional effort

Essential

  • oAbility to mobilise and stands for most of shift and kneels and crouches to deliver clinical care e.g. dress wounds.
  • oAbility to concentrate on providing clinical care such as drug calculations, carrying out tests and managing interruptions and unpredictable patient/client /carer behavior
  • oOccasional distressing or emotional circumstances such as imparting unwelcome news, care of terminally ill or safeguarding issues oPost holder could work in occasional unpleasant
  • oPost holder could work in occasional unpleasant conditions during home visits and could come across unpleasant body odours, dust, noise/ Body fluid or even verbal aggressions

  • oWork within clearly defined clinical and professional policies
  • oAbility to work independently seeking appropriate support and advice as necessary.

Knowledge and Skills

Essential

  • oLiterate in IT/Computer Skills
  • oEvidence of knowledge of clinical governance.
  • oKnowledge of the principles of Safeguarding of Vulnerable Adults and Children.
  • oKnowledge of the principles of health promotion and prevention of illness
  • oGood oral communication skills based on fluency in English language

  • oDemonstrate an understanding of national government initiatives in health care.

  • oEvidence of and commitment to multi-professional working
  • oUnderstanding of the interface between primary and secondary care.
  • oComprehensive understanding of the roles of others in health and social care
